We would like to invite your speciality pallitive care rogram to participate in a short survey to assess current practice of the collection of race, ethnicity, language (REaL) and sexual orientation and gender identity (SOGI) data on patients and clinicians in your program. The survey is estimated to take less than 10 minutes.  

With support of the California Health Care Foundation, the purpose of this project is to develop practical strategies to identify, collect, and report on REaL and SOGI data of patients and palliative care team members in service of identifying and reducing inequities in access to and quality of palliative care.

Identification of differences in access and quality is a prerequisite to achieving equity for our patients living with serious illness. Similarly, diversity in clinical staff is associated with better care quality. In both instances, improvement begins with measurement of what is.

The overarching purpose of this project is to develop practical strategies to identify, collect, and report on REaL and SOGI data on patients and palliative care team members through the PCQC Registry in service of identifying and reducing inequities in access to and quality of palliative care.
